Why do some flange bolts take a smaller wrench?
There are three common standards. DIN 6921 uses the larger wrench size; ISO 4162 and JIS B-1189 (Japanese) heads are smaller on some sizes, for example M8 is 13mm DIN and 12mm JIS. Check the wrench size listed with each item.

Do flange bolts need a washer?
Usually not. The flange works as a washer and spreads the load over a larger area.
